 METHODS OF TESTICULAR BIOPSY IN MALE INFERTILITY

  • TESA (Testicular Sperm Aspiration)

The method, which has been established itself as the most gentle method of sperm obtaining. The essence of this technique is described as the aspiration of testicular tissue through a thin needle inserted into the testicle through the skin, so that all conditioned segments of the testicle are biopsied. The contents of the syringe are immediately examined under a microscope for sperm searching.

If after 5-6 punctures of each testicle by the TESA method, no sperms are found, then a microsurgical variant of the puncture, TESE (Testicular Sperm Extraction), is used.

  • TESE (Testicular Sperm Extraction)

Assisted reproductive technology, the essence of which is to extract (aspirate) sperm from testicular tissue by needle biopsy of several testicular sites. It is an invasive method for sperm obtaining.

  • PESA (percutaneous epididymal sperm aspiration)

Percutaneous epididymal sperm aspiration (PESA) is a method used to determine the number of spermatozoa in case of possible of the vas deferens obstruction. Without dissecting the scrotum shells, during a needle biopsy, a specialist pierces the epididymis with the thinnest needle and sperm is extracted from it. Carrying out this manipulation due to the diagnosis of obstructive azoospermia guarantees the efficiency of the method to 100%.

It is an alternative to microepidymic sperm aspiration (MESA), and it aims to eliminate the technical difficulties and cost of MESA.

  • MESA (microsurgical epididymal sperm aspiration)

Manipulation consists of a single or multiple biopsy of the epididymis, and spermatozoa should be removed from the resulting fluid.

The MESA technique can only be used by a highly skilled microsurgeon, and therefore often is inferior to the PESA method.

TESA (for the diagnosis of non-obstructive azoospermia) and PESA (for obstructive azoospermia) are the gold standards for solving the problem of male infertility.
